What is the difference between monthly and quarterly payments?

With monthly payments you make 12 payments a year at one-twelfth of the annual rate per period, while quarterly payments are made 4 times a year. Both repay the same loan, but total interest differs slightly because of how often interest compounds against your balance.

Why does the first payment pay so little principal?

In an amortizing loan, each payment first covers the interest accrued since the last payment and the remainder reduces the principal. Early on, the balance is largest, so most of the payment goes toward interest; this reverses over time.
